Letter to GP & Medical Consent Form

This is a letter template asking for a medical report on an employee, together with the requisite consent form for the employee to sign.  If your Contracts of Employment are well drafted they should contain a provision giving you the right to seek advice from a qualified medical practitioner in the event that a particular employee has a medical condition that results in frequent sickness absences from work.  Making such a request assists an employer in discharging their duty of care for Health & Safety in the workplace and can also clarify whether the employee may have a disability within the meaning of the Equality Act.
